You will need to download and install the OmniForm Internet Publisher Plug-in ONE TIME ONLY in order to fill complete these forms.

This page of Instructions applies to Windows 95, 98, and Windows NT Workstation PC's.OmniForm does not work well with Macintosh computers, but there is a plugin for Macintosh below (choosing the pdf option for Macs is recommended over the omni option, the forms are the same).

(1) Download OIF32.exe by clicking on this link to the PLUG-IN. This will provide you with a Window indicating you have started downloading a file of unknown file type vms/exe. Macintosh Version of Omni form is in hqx format ( double click to install).

(2) You should then click on SAVE or SAVE FILE.

(3) Your PC system should then present you with another Window named SAVE AS where you may Choose t he folder/directory where the file will be stored. It is recommended you save the OIF32.exe file to the Desktop. Then click on SAVE and the file downloading should be complete.

(4) Before performing the next step, you should stop all windows programs you have running and close your browser (Netscape or IE).

(5) Double click on the OIF32.exe icon on your desktop (looks like ) to execute the program

(6) The extractor wi ndow will appear where you should click the SETUP button. Follow the instructions when prompted, accept defaults, agree to the license agreeme nt, and let the process complete, usually by clicking on NEXT, YES or FINISH.

(7) You will need to Close or Exit your Browser (Netscape or IE) before proceeding back to the ASU Forms Page. Once complete, this procedure should not require repeating.

(8) After installing the Omniform Filler Plug-In, you should not have to do anyth ing further except sign in to the ASU Forms System again and proceed. If you have installed the plug-in and are using Netscape Communicator but the form does not load properly from within the browser, make sure ALL Netscape windows are closed and restart Netscape.
